Safety is our top priority with our kids. When you come to pick up your child from VBS, please have photo ID ready and the pickup ticket that will be given to you at drop off.
If someone else is picking up, please notify the drop off coordinator and let them know that they will need their ID.
By checking this box, I understand that my child(ren) whose name(s) are listed on this form may be photographed or videoed at CCC during vacation bible school. I understand that these photographs or videos may be used in promotional materials, either in print or on social media/internet. By checking "I agree box", I grant permission for my child(ren) to be photographed, videoed, or their images to be recorded for print or electronic use in promoting CCC. I understand that it is my responsibility to update this form in the event I no longer wish to authorize the above uses. I understand that there will be no payment for me or my child's participation in this release.
By completing this form, I acknowledge that participation at Christ's Community Church Vacation Bible School may cause accidents or injuries and I hereby assume the risk of my child(ren) participating at Vacation Bible School. I also agree that as part of the consideration of entering and participating at Vacation Bible School to indemnity, release and hold harmless Christ's Community Church and its staff, elders, volunteers, from and against any and all loss, damage, injury, liability, and claims arising out of participation at Vacation Bible School at Christ's Community Church.
